Undertaking and Medical fitness Certificate-

1) Every Sport member have to pay non refundable registration fee Rs. 50/- along with membership Fee in Every academic year.

2) Yearly member can become member by paying one time yearly fee as per above mentioned

3) Maintain cleanliness in sport complex, Bath room & Toilet

4) Table tennis members should strictly follow their batch timings
-
5) Submit duly filled & signed membership application form to the department after registration and payment of fee.

6) All the members should understand that they they are under CCTV Surveillance.

Member should not have any contagious disease/communicable diseases for eligibility of membership

Member should not have any past Police/criminal record

1) Medical Certificate should be obtained from the registered MBBS / MD Doctor.

2) Right to acceptance of membership application is sports department & its decision.

3) New duplicate membership card will be provided on the payment of Rs 50/-.

4) Drinking & Smoking & gutka use is strictly prohibited in sport complex & is punishable.

5) Sport facility will be curtailed during the maintenance/technical problem/sport competition including public holiday and Sunday.

6) Bringing arms/ weapons in the sport complex is strictly prohibited & in that case membership will be permanently cancelled.

7) Kindly switch off, fan, lights if you are the last person to leave.

8) Gents & ladies batch will be separate in the morning & afternoon shift.

9) Use dustbin for dumping plastic bags/ waste papers etc. & should not be thrown in open space.
